Our nursing home transition program – also known as community reintegration – is available to those who have been living in a long term care facility and wish to move back into their community.

FITE CIL can help qualified individuals transition successfully into an independent life by providing financial support for housing, furnishings, medical equipment, accessibility remodeling and utility and rent down payments.

Another key to the success of the program is connecting individuals to the other services we offer, such as peer counseling and independent living skills training, in order to facilitate a successful transition to an independent lifestyle. We will also help you locate and teach you how to interact with an in-home personal assistant.

Eligibility
In order to be eligible for assistance through this program, an individual must:

• Have a disability
• Be age 59 or younger, OR
• Have a brain injury at any age
• Meet the criteria established by the Home Services Program
